Progress Notes Progress notes are the second genre most used in Dental Hygiene. These notes keep track of the patient’s improvements in their oral health. After each patent’s visit the hygienist is required to process a progress note including every detail of the patient’s oral health. This progress note is written in an informal tone and short handed. This progress note requires the patient name, time, date, reason they visited the clinic, the chief complain and the current oral health of the patient as well as his past medical history. The audience for this progress note is the Dental Hygienist, dentist and the patient or the patient’s guardian. Case Complains A case complains is the third most common genre in the field of a Dental Hygienist. Case complains are performed when a patient is experiencing pain. Within this case complain the hygienist need to write down the exact words of the patient and record all the information given. Along with the information given by the patient, the hygienist is required to attach the health history and the dental history to the case. This case complain also requires an oral cancer screening and vital sings of the day of the complain. This complain does not require a formal tone and the primary audience would be the dentist and in some cases the oral surgeon depending on the patients situation. |
Examination Reports
Examination reports are used for each patient from day one. This examination reports are intended to evaluate the state of a patient and diagnose abnormalities. These reports include examination of the lips, jaw, tongue, tonsils, dental occlusions, palets, swallow, breathing, oral habits, diadochokineses and tactile sensitivity. Examination reports vary from clinic to clinic but they all require the basic oral examination facts that where previously mentioned. Examination reports are the most important genres within dental hygiene, because they are the first step in preventing oral health diseases. These examination reports might also include dental X-rays or other diagnostic procedures. During an examination report the hygienist will likely discuss the patients diet and oral hygiene habits and demonstrate proper brushing and flossing techniques. Now in terms of filling out the examination report, the hygienist need to provide comments using proper terminology to insure her comment with its audience the patient and dentist. |
